Added mapping to DT_SURVEY.TASK_CODE in the SURVEY.xsd file of Alive Survey Definition formats.  Previously, TASK_CODE was present in the Alive Survey Definition formats, but was not mapped to the corresponding field in the DT_SURVEY table.
